Appointment No-Show, Cancellation & Service Policy:

I acknowledge and consent to the appointment and cancellation policy implemented by Aesthetics Collective MedSpa.

1. Appointment Scheduling:

I understand that it is necessary to schedule appointments in advance to receive services at Aesthetics Collective MedSpa. I will contact Aesthetics Collective MedSpa through the designated channels, such as phone, email, or online booking system, to schedule an appointment for the desired service(s).

2. Timeliness:

I acknowledge the importance of arriving on time for my scheduled appointments. I will make reasonable efforts to arrive at Aesthetics Collective MedSpa prior to the scheduled appointment time to allow for necessary paperwork, consultation, and preparation.

3. Cancellation and Rescheduling:

I understand that circumstances may arise that require me to cancel or reschedule my appointment. In such cases, I agree to provide Aesthetics Collective MedSpa with advance notice to cancel or reschedule an appointment. I acknowledge that a non-refundable $30 deposit is required to secure all appointments valued at $50 or more, and that this deposit will be applied toward my total service cost at checkout.

4. Cancellation Fees:

Appointment Cancellation Policy: A non-refundable $30 deposit is required to secure all appointments valued at $50 or more. This deposit will be applied toward your total service cost at checkout. Failure to provide timely notice of cancellation may result in forfeiture of the deposit.

5. No-Show Policy & No-Show Fees:

I acknowledge that failing to show up for a scheduled appointment without prior notice will be considered a "no-show." I understand that repeated instances of no-show may result in the termination of my client relationship with Aesthetics Collective MedSpa. No-shows will forfeit the $30 deposit. A no-show fee of $50 may be charged for any complimentary consultations scheduled.

6. Late Arrival:

I acknowledge that arriving late for a scheduled appointment may result in a shortened or modified service. In such cases, I understand that I will still be responsible for the full payment of the scheduled service.

7. Confirmation and Reminders:

I understand that it is my responsibility to confirm the date, time, and location of my scheduled appointment(s) with Aesthetics Collective MedSpa. I acknowledge that Aesthetics Collective MedSpa may provide appointment reminders through various communication channels, such as phone calls, emails, or text messages, but that it is ultimately my responsibility to manage and remember my appointments.

8. No Refunds Policy:

All services and products provided by Aesthetics Collective MedSpa are non-refundable. By purchasing our services or products, you agree to these terms. In the event of dissatisfaction, we encourage you to reach out to our team to discuss potential solutions, including rescheduling or providing alternative options where applicable. Please note, this policy is in place to maintain the quality and integrity of our services and products

By signing below, I confirm that I have read, understood, and agreed to the terms of the Appointment and Cancellation Policy implemented by Aesthetics Collective MedSpa. I accept that it is my responsibility to adhere to this policy and manage my appointments and cancellations accordingly.
