I understand that it is necessary to schedule appointments in advance to receive services at Aesthetics Collective MedSpa. I will contact Aesthetics Collective MedSpa through the designated channels, such as phone, email, or online booking system, to schedule an appointment for the desired service(s).
I acknowledge the importance of arriving on time for my scheduled appointments. I will make reasonable efforts to arrive at Aesthetics Collective MedSpa prior to the scheduled appointment time to allow for necessary paperwork, consultation, and preparation.
I understand that circumstances may arise that require me to cancel or reschedule my appointment. In such cases, I agree to provide Aesthetics Collective MedSpa with advance notice to cancel or reschedule an appointment. I acknowledge that a non-refundable $30 deposit is required to secure all appointments valued at $50 or more, and that this deposit will be applied toward my total service cost at checkout.

A no-show fee of $50 may be charged for any complimentary consultations scheduled.

I acknowledge that Aesthetics Collective MedSpa may provide appointment reminders through various communication channels, such as phone calls, emails, or text messages, but that it is ultimately my responsibility to manage and remember my appointments.
All services and products provided by Aesthetics Collective MedSpa are non-refundable. By purchasing our services or products, you agree to these terms. In the event of dissatisfaction, we encourage you to reach out to our team to discuss potential solutions, including rescheduling or providing alternative options where applicable.
